{"id":63631,"date":"2026-09-11T00:00:00","date_gmt":"2026-09-10T23:00:00","guid":{"rendered":"https:\/\/www.desiblitz.com\/jobs\/job\/patient-pathway-coordinator\/"},"modified":"2026-09-11T01:29:42","modified_gmt":"2026-09-11T00:29:42","slug":"patient-pathway-coordinator","status":"publish","type":"job_listing","link":"https:\/\/www.desiblitz.com\/jobs\/job\/patient-pathway-coordinator\/","title":{"rendered":"Patient Pathway Coordinator"},"content":{"rendered":"<p>Job Title: Patient Pathway Coordinator<\/p>\n<p>Location: Craven Road, Reading, RG1 5AN<\/p>\n<p>Hours: 37.5 hours per week \u2013 Monday \u2013 Friday mixture of shifts 8am \u2013 4pm &amp; 9am \u2013 5pm<\/p>\n<p>Trust: Royal Berkshire Healthcare NHS Foundation Trust\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0<\/p>\n<p>Role type: Temporary for 3 months with possibility of becoming permanent<\/p>\n<p>Reference: 91057<\/p>\n<\/p>\n<p>Role details<\/p>\n<p>The post holder will be the point of contact for all administrative issues relating to patients\u2019 pathway of care. The post holder will work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The post holder will supervise and allocate work to the PPC team.<\/p>\n<p>Duties &amp; responsibilities<\/p>\n<ul>\n<li>Process and log all referrals, including paper and Choose and Book, in line with Trust Access &amp; Administration Process Policy and speciality SOP<\/li>\n<li>Schedule new and follow up appointments, ensuring that capacity is proactively and efficiently used. This includes booking patients into the right clinic to ensure that they are seen by the most appropriate clinician first time and for subsequent follow ups<\/li>\n<li>Ensure that any diagnostics and investigations that are required as part of the patient pathway have been requested and that the results are available for consultations. This will include those that are carried out at other Trusts<\/li>\n<li>Co-ordinate appointments and procedures at other hospitals and organisations, where the pathway requires input from these<\/li>\n<li>Ensure outcomes of clinic attendance have been accurately recorded on EPR by the Patient Pathway Assistant (\u2018cashing-up\u2019).
