We are recruiting on behalf of a well-established and highly regarded housing organisation for an Intensive Housing Officer to join its Adult Supported Housing team.<\/p>\n
This is an excellent opportunity for someone with experience in supported housing, intensive housing management, housing-related support or community-based support, who is passionate about helping adults with mental health needs, learning disabilities and other complex needs maintain their independence, wellbeing and tenancy.<\/p>\n
You will provide structured, person-centred and outcome-focused support to residents, ensuring that support is delivered safely, consistently and in line with agreed support plans and organisational procedures.<\/p>\n
The role<\/p>\n
As an Intensive Housing Officer, you will:<\/p>\n
Deliver planned Intensive Housing Management (IHM) and housing-related support to residents.<\/p>\n<\/li>\n
Support adults with mental health needs, learning disabilities and other complex needs.<\/p>\n<\/li>\n
Contribute to assessments, support plans, reviews and resident outcome monitoring.<\/p>\n<\/li>\n
Maintain accurate and timely case notes, contact records and support evidence.<\/p>\n<\/li>\n
Promote independence, tenancy sustainment, engagement and positive resident outcomes.<\/p>\n<\/li>\n
Identify and appropriately escalate safeguarding concerns, changes in risk, incidents and welfare concerns.<\/p>\n<\/li>\n
Work closely with residents, families, colleagues, care providers and external partner agencies.<\/p>\n<\/li>\n
Attend service locations, local authority meetings, partner meetings and stakeholder forums as required.<\/p>\n<\/li>\n
Use case-management and ICT systems effectively while maintaining high standards of data protection and confidentiality.<\/p>\n<\/li>\n
Follow and embed organisational approaches to resident journeys, recording standards and service improvement.<\/p>\n<\/li>\n<\/ul>\n
About you<\/p>\n
We are looking for someone who has:<\/p>\n
Previous experience in supported housing, housing-related support, Intensive Housing Management or community-based support.<\/p>\n<\/li>\n
Experience supporting adults with mental health needs, learning disabilities or other complex needs.<\/p>\n<\/li>\n
Experience maintaining high-quality case records, support notes, risk information and outcome evidence.<\/p>\n<\/li>\n
A good understanding of safeguarding, risk management and tenancy sustainment.<\/p>\n<\/li>\n
Experience working collaboratively with residents, families, colleagues and external agencies.<\/p>\n<\/li>\n
Strong communication and relationship-building skills.<\/p>\n<\/li>\n
The ability to work independently, organise your workload and manage competing priorities.<\/p>\n<\/li>\n
Good IT and case-management skills, with a strong focus on accurate record keeping.<\/p>\n<\/li>\n
A person-centred, strengths-based approach to supporting residents.<\/p>\n<\/li>\n<\/ul>\n
A Level 3 qualification in housing, health and social care, community support or a related discipline would be desirable, although equivalent relevant experience will be considered.<\/p>\n
Additional qualifications or experience in CIH, safeguarding, mental health, health and social care, supported\/exempt accommodation or tenancy sustainment would also be advantageous.<\/p>\n
